Quick answer: what are All-on-X dental implants?

All-on-X dental implants are a full-arch treatment in which one fixed bridge replaces every tooth in the upper or lower jaw and is anchored to a set number of implants, the "X", usually between four and eight, chosen after a 3D scan of your bone, bite and jaw.

Only the dentist can remove the bridge, and many patients leave surgery with fixed temporary teeth.

Medical note

This page is general information, not a diagnosis. Whether All on X implants suit you, and how many you need, is decided by a dentist after an examination and a 3D (CBCT) scan. Always tell us about diabetes, smoking, past gum disease, head and neck radiotherapy, blood thinners and osteoporosis medicines.

What does the "X" in All-on-X mean?

The "X" in All-on-X is simply the number of implants that hold the full-arch bridge in one jaw: All-on-4 uses four, All-on-6 uses six and All-on-8 uses eight.

A dental implant is a small titanium post placed in the jawbone that acts as an artificial tooth root. In an all on x case the implants share one continuous bridge, usually carrying ten to twelve teeth. Cleveland Clinic notes that a full arch is usually supported by four to six implants, and some plans use more when the bone allows it.

All-on-X is not a brand or a special implant; it is a treatment family, and the right X depends on your bone, jaw and bite. If you only need one or a few teeth, a single implant or implant bridge may fit better.

The All-on-X family: choose the page that matches your case

All-on-4

Four implants per jaw, with the back two usually tilted to make the most of existing bone.

All-on-4 dental implants in Turkey

All-on-6

Six implants per jaw, spreading the bite over more support when bone volume is good.

All-on-6 dental implants in Turkey

All-on-8

Eight implants per jaw, often considered for the softer upper jaw when there is enough bone.

All-on-8 dental implants in Turkey

Full mouth

Both jaws restored, upper and lower, often combining two All-on-X bridges in one plan.

Single implant (1 tooth)

Not part of All-on-X: the alternative when only one tooth is missing, with one implant and one crown.

Single tooth implants

Multiple implants

Several missing teeth in a row replaced with implant-supported crowns or a short bridge.

All-on-4 vs All-on-6 vs All-on-8: how do the options compare?

All-on-4, All-on-6 and All-on-8 all give you fixed teeth on one bridge; they differ in how many implants share the load and how much healthy bone each one needs.

More implants are not automatically better: each one needs its own healthy bone, so the plan follows your anatomy.

What if I have bone loss?

Bone loss does not always rule out All on X implants, because tilted implants, bone grafting, a sinus lift or, in severe upper-jaw cases, zygomatic implants can be options. In All-on-4 the two back implants are tilted up to about 45 degrees to use the bone that is there. A 2021 systematic review of 25 studies in the Journal of Indian Prosthodontic Society found no significant difference in survival between tilted and straight implants, with implant survival of about 94.8 to 98 percent over six to eleven years; none of the studies was a randomised trial. For a severely resorbed upper jaw, a 2021 overview reported zygomatic implant survival of 95.2 to 100 percent, with sinusitis the most common complication. Who is a good candidate for All-on-X?

A good candidate for All-on-X is an adult who has lost all or nearly all teeth in one jaw, or whose remaining teeth cannot be saved, and who has enough bone (or can have it rebuilt), healthy gums and no illness that blocks bone healing.

  • Usually suitable: fully grown jaw, enough bone or a grafting plan, healthy gums and time for a treatment lasting several months.
  • Needs a closer look: smoking, poorly controlled diabetes, active gum disease, radiotherapy to the head or neck, or medicines that affect bone.
  • Common reasons: a loose denture or many failing teeth.

Mayo Clinic notes that smoking may contribute to implant failure. If you are unsure, our page on when dental implants cannot be placed explains the main limits.

How does All-on-X treatment work in Istanbul, step by step?

All-on-X treatment in Istanbul usually happens in two trips: the first for surgery and fixed temporary teeth, the second for the final bridge after the implants have healed into the bone.

  1. Remote first opinion: share your X-rays and medical history on WhatsApp.
  2. Examination and 3D CBCT scan: in Istanbul the dentist checks your mouth, reviews your health and takes a 3D scan of the jaw.
  3. Digital planning: the position, length and angle of each implant are planned on the scan, with a surgical guide when useful.
  4. Extractions and implant placement: teeth that cannot be saved are removed and the implants are placed under local anaesthesia.
  5. Fixed temporary bridge: if the implants are stable enough, a temporary bridge is screwed in shortly after surgery. In the All-on-4 review above, every included study fitted it within 48 hours.
  6. Healing at home: the implants fuse with the bone over several months while you eat a softer diet and send check-in photos.
  7. Final bridge: on the second trip the team scans, tries in and fits the definitive bridge.
  8. Follow-up: regular check-ups and professional cleaning.

How long does All-on-X take?

Cleveland Clinic gives about three to six months for implants to fuse with the jawbone, longer after bone grafting, and says most people feel normal about one week after implant surgery.

Trip 1: about 5 to 7 days (indicative, clinic estimate)

Examination, CBCT, planning, surgery, fixed temporary teeth and a check before you fly home.

About 3 to 6 months

Implants integrate with the bone while you are at home, with remote support from the team.

Trip 2: about 5 to 7 days (indicative, clinic estimate)

Scans, try-ins and fitting of the final bridge, plus hygiene instructions.

Trip lengths are indicative; see also how many days you need in Turkey for dental implants.

Which material is used for the final All-on-X bridge?

The final All-on-X bridge is usually either a hybrid with acrylic teeth on a metal framework or a zirconia bridge milled from a solid block, and each has its own pattern of wear and repair.

PointAcrylic hybrid (metal + acrylic)Zirconia bridge
BuildMetal frame with acrylic teeth and pink gum sectionHigh-strength ceramic, often milled in one piece
Common issuesAcrylic teeth can wear or fracture, mainly in people who grindFewer minor complications such as chipping in the comparison study
Prosthesis survival in the comparison study83.0 percent93.7 percent (difference not statistically significant)
Inflammation around implantsNo significant difference between groupsNo significant difference between groups

A 2021 systematic review in Materials found only one study comparing zirconia directly with metal-acrylic, so the choice is made case by case with your dentist. For crown materials, compare zirconia bridges and E.max crowns.

How do you care for All-on-X implants?

You care for All-on-X implants inside the mouth every day: brush at least twice daily with a soft brush, clean under the bridge with interdental brushes or floss threaders, and keep regular check-ups and professional cleanings.

  • Brushing: soft-bristled brush and non-abrasive toothpaste, as Cleveland Clinic advises for fixed implant dentures.
  • Under the bridge: interdental brushes or floss threaders.
  • Food: soft food while healing; avoid biting ice later.
  • Habits: ask about a night guard if you grind, and stop smoking.

Cleveland Clinic notes that the denture on the implants should be replaced every 15 to 20 years. See also how long dental implants last.

What are the risks of All-on-X implants?

All-on-X is surgery, so possible risks include infection, nerve injury causing numbness, sinus problems in the upper jaw, an implant that fails to fuse, bridge fracture and, over the years, peri-implantitis.

Mayo Clinic says a failed implant can be removed and placement retried after about three months. The American Academy of Periodontology separates peri-implant mucositis, inflammation of the gum only that can be reversed if caught early, from peri-implantitis, where the supporting bone is lost and surgery is usually needed. Past gum disease, poor plaque control, smoking and diabetes raise the risk.

Contact your dentist soon if you notice:

  • Bleeding, redness or swelling around the implants
  • Pain that gets worse after the first few days
  • Pus or a lasting bad taste
  • A bridge or implant that feels loose
  • A crack or broken tooth on the bridge

All-on-X FAQ

What is All-on-X?

All-on-X is a treatment that replaces all the teeth in one jaw with a single fixed bridge anchored to a set number of dental implants. The X is that number, usually between four and eight.

Is All-on-X the same as All-on-4?

All-on-4 is one member of the All-on-X family. It uses four implants per jaw, while All-on-6 and All-on-8 use six and eight. The idea is the same; the number of supporting implants differs.

How many implants will I need?

The number depends on your bone volume and quality, which jaw is treated and your bite force. The dentist decides after a clinical examination and a 3D CBCT scan.

Can I have All-on-X if I have bone loss?

Often yes. Tilted implants in All-on-4 can use existing bone, and bone grafting, a sinus lift or, in severe upper-jaw cases, zygomatic implants may be options. Your scan shows what is possible.

Will I leave Istanbul with fixed teeth?

In many cases yes. If the implants are stable enough on surgery day, a fixed temporary bridge is screwed in shortly afterwards. The final bridge is fitted after healing.

How long does All-on-X take in Turkey?

Most patients make two trips to Istanbul. As an indicative clinic estimate each trip takes about 5 to 7 days, with roughly three to six months of healing at home in between.

Is All-on-X better in zirconia or acrylic?

Both are used. In one comparison study, metal-acrylic bridges had more minor problems such as chipping and tooth fracture, while survival was not significantly different. Your dentist will advise based on your bite and habits.

Does All-on-X surgery hurt?

The implants are placed under local anaesthesia. Swelling and soreness are common in the first days, and most people feel normal after about one week.

How do I clean an All-on-X bridge?

Brush at least twice a day with a soft brush and non-abrasive toothpaste, clean under the bridge with interdental brushes or floss threaders, and attend regular check-ups and professional cleanings.

What if I am only missing one tooth?

All-on-X is for a full arch. If you are missing one tooth, a single implant with one crown is the usual alternative, and several missing teeth can be replaced with multiple implants or an implant bridge.
